Nuceiva is indicated for the temporary improvement in the appearance of moderate to severe glabellar lines in adult patients < 65 years of age.

Nuceiva Terms and Conditions:

• Nuceiva vials must be refrigerated at 2°- 8° Celsius immediately upon receipt.
• Interval between deliveries of an automatic shipment schedule cannot exceed 1 calendar month.
• Invoicing will follow the delivery.
• You may end your automatic renewal subscription agreement after a minimum period of six (6) months, subject to the applicable conditions.*
• Your Nuceiva delivery will arrive in a re-usable CREDO container. The CREDO container must be returned to the driver.
• If a customer fails to return the CREDO container to the driver at the time of delivery, they must follow the enclosed return instructions to do so as soon as possible (pre-filled waybill is included).
• If the CREDO container is not returned, a $500 charge for the replacement cost of the container will be invoiced.
• Payment terms based on approved credit.
• All products are non-refundable and non-exchangeable.
• Nuceiva cannot be shipped over the weekend, therefore Monday deliveries are not available.
• Allow extra time for FALL AND WINTER shipping.
